Do I need the DS-11 or the DS-82?

Use the DS-82 to renew by mail only if all six conditions are true: you can submit your most recent passport, it has never been reported lost or stolen, it was issued within the last 15 years, it was issued when you were age 16 or older and was valid for 10 years, it is in your current name or you can document the change, and it is undamaged beyond normal wear and tear. If any one of those is not true, you apply in person on the DS-11.

Can I sign my DS-11 at home?

No. The State Department instruction is “Do not sign your form until asked to do so by a passport acceptance agent.” Fill the form in, print it, and sign it at the counter. Simplicity AI leaves that signature blank on purpose.

Can I renew my passport online instead?

Sometimes. The State Department says to renew online if you are eligible and are applying for routine service. The DS-82 route by mail stays available and is the one that covers expedited service, so which suits you depends on how soon you are traveling.

What does my child need?

Bring the child to an authorized passport acceptance facility in person, and both parents or guardians must be present. Passports for children under age 16 are valid for 5 years, against 10 years for an adult, so children come round again sooner than the rest of the household.

What if one parent cannot come to the appointment?

That parent completes Form DS-3053, and it has to be signed and dated in front of a certified notary public. It is then submitted with the child’s DS-11. If one parent cannot be located at all, Form DS-5525 covers that situation instead.

My passport was lost or stolen. What do I file?

Form DS-64 reports it, and you apply in person on a Form DS-11 to replace it. A passport that has been reported lost or stolen can never be renewed by mail afterwards, because never having been reported lost or stolen is one of the six DS-82 conditions.

I changed my name. Which form?

Two routes exist. Form DS-5504 covers name changes, data corrections, and replacing limited-validity passports, and you sign and date it after printing. The DS-82 also accepts a name change if you can provide a document such as a marriage certificate, divorce decree, or court order. Which one applies depends on your passport’s history, so answer the questions rather than guessing.
